Archie Hendricks Sr. Skilled Nursing Facility (AHSSNF) is a Medicare/Medicaid-certified, 60-bed facility. We operate under the direction of the Tohono O’odham Nursing Care Authority (TONCA), providing a high level of nursing care for our elders. Our licensed and certified nursing staff provides high-quality healthcare services 24/7/365, as well as supporting departments that assist in meeting the needs of residents. AHSSNF is a short-term and long-term skilled nursing care facility that is strategically located in the center of The Nation for the convenience of our residents and their loved ones. Because our facility is located within the Tohono O’odham Nation, elders who previously would have had to go to off-reservation facilities can now stay close to home and receive care that is aligned with our traditions and values.

Who Needs Skilled Nursing Care (Short-Term Care) at AHSSNF?

Skilled nursing care is also referred to as short-term care and is not the same as assisted living care for elders. Short-term care is recommended by a person’s doctor and once the admission is approved, the person who arrives at AHSSNF has the goal of returning home as soon as they are clinically cleared. Here, our compassionate and highly trained staff members care for the elders of our Nation who have acute injuries or illnesses. Many elders can benefit from our skilled nursing care, including those in the following situations:

  • Those who have been recently discharged from a hospital, yet are still in need of medical care before returning home, such as:
    • Those who have fallen and sustained fractures
    • Those who have suffered a stroke and require rehabilitation
    • Those who require round the clock medical monitoring
    • Those who have wounds that are difficult to take care of at home
    • Those who need IV therapy
    • Those who need Physical and/or Occupational Therapy

Please note that at this time, AHSSNF is only able to accept persons who have been referred for short-term skilled nursing care by another facility.

Who Needs Long-Term Care at Archie Hendricks Sr. Skilled Nursing Facility (AHSSNF)?

A person may need long-term care if they are not able to take care of themselves at home due to physical or cognitive limitations or chronic health conditions that are expected to continue for an extended period of time. For the long-term care resident, AHSSNF provides a wide range of services that include the following:

  • Activities of daily living – Assistance with bathing, grooming, and other daily activities
  • Physical and occupational therapy – As ordered by patients’ primary care providers and provided by our licensed staff
  • Wound care – Specialized care for slow-healing wounds
  • IV therapy – Such as antibiotic therapy as ordered by patients’ primary care providers
  • Traditional medicine – Upon patients’ request and delivered by a medicine person of their choosing
  • Telemedicine – Virtual visits for residents with their primary care provider
  • Behavioral health services – Available through virtual and in person visits
  • Respite care – Temporary care to provide relief for the caregiver(s)
  • Palliative healthcare – Any health services designed to ease symptoms, rather than to have a curative effect
  • Hospice care – Contracted through Compassus of Casa Grande

 

Short-term Care and Long-term Care residents at AHSSNF and their family members benefit from the full support of our caring team. We are pleased to offer a range of culturally oriented supportive services, including the following:

  • Transportation (for both medical appointments and activities)

For referring facilities, AHSSNF requires the following paperwork:

  • Face Sheet, including Social Security number and payor source (insurance)
  • Immunization History
  • Height and weight
  • Fall History past 90 days
  • Current TB clearance
  • Current progress notes from referring facility
  • Current PASRR to be filled out by referring facility
  • Current Labs
  • PT/OT evaluations and weight-bearing (if applicable)
  • Wound care treatment orders and notes (if applicable)
  • Special medical supplies if needed (bariatric bed and other bariatric items, special mattresses, wound vac, etc.)
  • Physician’s order for special meds
  • Nurse-to-nurse report
  • Doctor-to-doctor report
